For support coordinators, families & health professionals
We work closely with NDIS support coordinators, Local Area Coordinators (LACs), plan managers, allied health teams and hospital discharge planners — as well as participants themselves, family members and carers. Whatever your role, our intake team will make the referral simple and keep you informed.

You can refer without every detail, but the more you can share, the faster we can confirm a good match:
- The participant's goals and the support they're seeking
- Plan management type — self, plan or NDIA-managed

After you submit a referral
1. We review it
Our intake team personally reviews every referral and checks we're the right fit for the participant's goals, plan and location.
2. We get in touch
We contact you (and the participant, with consent) to talk through supports, answer questions and confirm availability.
3. Support begins
We set up a clear service agreement, match a suitable support worker and start — with regular check-ins to keep things on track.
